Thank you for your interest in becoming a member of our Eucharistic Family!

There are a number of ways to register:

• Pick up a Parish Registration packet, found at any of the three entrances of the church. Complete the Parish Registration form, then mail it to us, drop it in the collection basket at Mass, or bring to the the Rectory Office located at 3450 Norwood Rd. across the street from the church.
• Visit us at the Rectory Office, (located at 3450 Norwood Rd. across the street from the church), and complete the Parish Registration form in our office.

• Click here to save a copy of the Parish Registration Form (PDF)  to your computer and print out a copy.  You can email or mail the completed form to us, drop it in the collection basket at Mass, or drop it off at our Rectory Office located at 3450 Norwood Road across the street from the church. • OR, 

1. Download the Parish Registration form:  click here for Parish Registration Form (pdf).
2. Save the Parish Registration Form to your computer. Please note you must save the Form to your computer first before completing. (NOTE TO MAC USERS:  do not save to Mac “Preview”; change the setting to “Adobe Acrobat”.)
   
3. Open the saved Parish Registration Form from your computer and fill out the information in the highlighted fields directly on the form.
4. Press the “Submit” button at the bottom of the form when complete.
5. Your email application should open with your completed form attached to an email message addressed to us.
   If your email application does not open automatically, please click here:parishoffice@stdominicchurch.net and attach your saved form (pdf) to your email message
